Operation has not done any fire drills in 2025. CCR observed an effective fire drill during inspection.

CPSC signed notification has not been completed since 9/2022.

It was determined one caregiver's First Aid/CPR certification had expired.

Observed a caregiver yelling at two children to stop.

Observed the emergency practices for fire drills was not conducted in February or May. This was corrected when a fire drill was completed during the inspection.

Based on information obtained during an investigation, it was found that a caregiver inappropriately restrained a child in care, preventing the child from moving, due to the child not wanting to nap.

Based on information obtained during an investigation, it was found that a caregiver failed to use courtesy, respect, acceptance, and patience when performing assigned responsibilities with a child in care when the caregiver stated, "I'd like to be in a room with no witnesses".

During an investigation, it was determined a child was left in a tight and dirty diaper leading to a diaper rash.

After a review of records, it was determined there was no annual training regarding the following required content: administering medication nor preventing or responding to a food or an allergic reaction.

During an inspection, it was determined that a fire drill was not completed nor documented for the previous month. A fire drill was completed and documented during the inspection, bringing the facility into compliance.
