A caregiver was briefly left alone with thirty seven children while the second caregiver took a child to the bathroom.

During the course of a DFPS investigation, it was determined that multiple caregivers failed to demonstrate good judgement in the execution of their duties. The caregivers did not follow the operation's established protocol for accounting for children following a field trip. They failed to do a count of children, did not conduct a name to face count, and failed to conduct a full walkthrough of the bus which resulted in a child being left on a bus alone for approximately 15 minutes.

During the course of a DFPS investigation, it was determined that caregivers failed to account for all children, resulting in a child being left on the bus alone. The vehicle was returned to another operation location, where the child was left on the bus for approximately 15 minutes. Staff were unaware of the child's absence from the group until the child's parent came to pick them up from care.

The operation allowed a person to be present and have access to children without an active background check.

This standard was found deficient as part of a DFPS Investigation.

Based on information gathered, supervision on the playground was not appropriate, which led to a group of children fighting for approximately seven to eight minutes.

Based on information gathered, employees did not know who was in charge.

The operation failed to timely notify Child Care Regulation of an incident that placed children at risk.

One personnel file lacked orientation to the operation. Note: This was corrected when the employee provided a signed statement stating he received orientation to the operation.

The first aid kit lacked a guide to first aid.

One out of four personnel files evaluated lacked an affidavit.
