A child with a listed allery had medication on file that had expired 2/2025. NOTE: Medication was returned to parent when child was picked up.

A child with a listed food allergy and medication on site did not have a food allergy plan on file.

This standard was found deficient as part of a DFPS Investigation. Caregivers failed to accurately account for a child who was permitted to move between group activities resulting in the child being able to exit the childcare space and building.

This standard was found deficient as part of a DFPS Investigation. Caregivers failed to communicate to one another by verifying which caregiver had assumed responsibility of a child while the child was permitted to enter and exit the building between group activities.

This standard was found deficient as part of a DFPS Investigation. A child with a history of wandering and diagnosed special needs, was not appropriately supervised which allowed the child to leave the cafeteria without caregiver knowledge.

At least 9 children were observed going to the restroom without informing caregivers or being monitored by a caregiver. The restrooms are not in direct sight of the program staff. 1 child was observed washing their hands in the water fountain.

A child with a food allergy plan had an epipen at the operation without a prescription box or label.

The operation has not obtained a missing food allergy plan for a child enrolled in the operation. The operation was not aware a child enrolled had a food allergy plan on file and the plan on file was dated 8/2019 .
