Two children were not properly supervised when the children were able to get outside of the building without the knowledge of operation staff. The children were found outside in front of the building several minutes later by a caregiver. A caregiver left two children unsupervised in the restroom as the caregiver failed to properly account for all children prior to transitioning the children back to their classroom. The children were left unsupervised in the restroom for several minutes before being discovered by another caregiver. The caregiver in charge of the group was not aware that the two children were not with the rest of the group.
