During the inspection, the parent handbook was viewed and missing information about the hours, days and months of operating, information on suspension and expulsion of children, safe sleep, parents' rights, preventing and responding to abuse and neglect of children, information on the operation maintaining and administering epinephrine pen, information on parents' rights, information on supporting inclusive services to children with special needs and information on gang free zone

During the inspection the sanitation inspection was expired on 12/13/2024

During the monitoring inspection it was observed that a four month old infant was observed sleeping in a bouncer seat, wrapped in a blanket.

During the inspection it was observed that all of the classes were scheduled to go outside at 10:30am and none of them went out. Two of the classes did go after I asked them about their reasoning for not going.

During the inspection it was observed that the operation had two slides, both under 5 feet, that were placed on grass without loose fill.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One caregiver was observed in the classroom without an eligible background check. The assistant director removed caregiver from the classroom.

During inspection, permit holder is not ensuring the operation is maintain compliance with minimum standard rules.

During follow up inspection,the unlock/lock knob on the van is broken.

During inspection, one of the televisions observed was not anchored or mounted.

During inspection, caregivers hired on 1/4/2024 and 10/9/2016 did not know the ages of the child they were providing care for.

Operation has not maintained or updated admission forms for children in care.

During inspection, one of the two playground observed did not have any mulch for equipment less than 5 feet.

During inspection, potty chair used by child was observed on the floor of the bathroom, the unlock/lock knob and back door on the van is broken.

During inspection, van did not sound the child safety alarm.

During inspection, peanut butter that exipred on 10/2023 was present in pantry.

During inspection, one of the two playground observed did not have any mulch for equipment greater than 5 feet.

Two caregivers were observed outside of the infant classroom leaving students unsupervised.

Director is missing 30 hours of annual training for 2019-2020 year.

During inspection, ointment, bandaids, and antiseptic wipes were expired in van.
