During the inspection walk through, caregivers were observed washing bottles and lunch dishes in the handwashing sink.

Several bottes in the three infant classrooms were not labeled with the chlid's name. Caregivers labeled the bottles during the inspection.

The last certification was completed 1/2020. This was corrected when the director completed a new certification during the inspection.

A new van used for transport of children did not have a child safety alarm installed.

One fire extinguisher on a van used for transport was not serviced.

On all the transportation vans, garbage, food trash, and debris were found int he seats and floors of the vans.

2 vans used for transport of children did not have working child safety alarms.

All children being transported in all 4 vans did not have emergency medical authorization information for transport.

All transportation vans did not have thermometers in the required first aid kits. This was corrected when the director added thermometers to all the first aid kits in the vans.

Based on the information gathered a teacher witnessed a child left alone on the playground with no daycare staff present.

Based on the preponderance of evidence, the caregiver left a child unattended in a classroom's restroom without supervision.

Based on the preponderance of evidence, the caregiver in care of a group of children did not know the number of children in their care when they transitioned from the classroom to the van to transport to school as one child was left in the classroom.

Based on the information obtained during the investigation, there is sufficient evidence to support that a child was left unattended on the playground for at least 1 1/2 minutes.

A caregiver's personal bag and drink was accessible to children. The caregiver's bag was removed when instructed.

The children in the 2 year old classroom did not wash their hands after outdoor play.

One of the electrical outlets was accessible to the children in one of the Toddler rooms. The outlet was covered during the walk through.

Based on the information obtained during the DFPS investigation a child trips over a toy and the caregiver does not promptly go to the child to comfort and check on them even though the child was crying.

Based on the information observed during the DFPS investigation the caregiver was seen on video involved in sweeping the floor while she had children in her care and as a result was not properly supervising the children.

Based on the information obtained during the DFPS investigation a caregiver was observed roughly handling a child by picking them up and placing them down on the carpet in a manner that caused the child to fall backwards.

In 2 classrooms there was garbage that was accessible to children.

2 out of 9 files evaluated were missing the additional required training topics.

The last documented fire inspection was on 8/2021.

Based on the information obtained during the investigation there is sufficient evidence to show that a caregiver did not use positive methods of discipline and guidance when grabbing a child?s arm and twisting the child?s arm to redirect a child from doing inappropriate actions.

A caregiver in the three year old classroom hit a child as a form of discipline. Note: Corrected when the caregiver was immediately terminated.

Based on information obtained, the operation evacuated the building during a fire drill, but left one toddler behind.

The operation does not have a current annual fire inspection.

The operation failed to make the necessary corrections in the fire inspection report.
