Based on the evidence obtained during the investigation, multiple caregivers, some seated with their backs to majority of the children in care, lacked visual awareness and they were not aware of the ongoing activities of a child in care throwing pieces of a plastic balance beam repeatedly that contributed to another child in care becoming injured.

A caregiver was observed changing diapers and did not wash their hands after diaper changes.

Medication that is related to a food allergy plan was not in the original container with the child's information and also has expired and requires being disposed and replaced.

Documentation of emergency practices drills, including monthly checks for the fire extinguisher, smoke detector and carbon monoxide detector, was not available for review at inspection.

The toddler middle classroom was observed during naptime and there was no lighting being used and eyes had to adjust. The caregivers and children were not able to be observed in the classroom until a flashlight was used during the classroom walkthrough.

Two children's files who had diagnosed food allergy plans. One child did not have a complete food allergy plan and the other was missing the parent signature.

During the inspection, a child was observed drinking a bottle of milk while the entire classroom was in the restroom on a restroom break/diaper changes were being conducted in the restroom. The child was observed drinking the bottle leaning on a sink while standing on a small step stool and then was moved to the diaper changing table where a caregiver changed the child's diaper as the child continued to drink from the bottle. The restroom was not a sanitary place for the child to be drinking the bottle.

Two separate caregivers were observed not sanitizing the changing tables after diaper changings two children back to back each. One changing pad used was torn and unable to be sanitized appropriately.
