During follow up, there are 2 caregivers who are missing annual training for 2024-2025 training year.

During follow up inspection 3 caregivers files did not have Pre-Employment Affidavit Form 2912.

During follow up inspection 6 caregivers did not have a notarized affidavit Form 2985 in their employee files.

At time of follow up at 7:00 am to 7:22 am infants were still observed in highchairs.

During the inspection it was found that one staff member had taken CPR training online.

After two infants were observed done eating the infants were observed sitting in a high chair for an additional 15 minutes.

At time of inspection, children were observed watching television for over an hour.

One out of the three van's child safety alarm did not work.

At time of inspection, one caregiver had 18 children. Ages ranging from 12 months to 9 years old.

At time of inspection, I observed a child open a locked door for an unknown person. Both caregiver and director were located in two separate rooms and could not see who was at the door. Caregiver also left the children unsupervised to open the door on two occasions.

At time of inspection, the children were observed watching a movie called Hotel Transylvania.

During inspection, children were observed watching television. Based on daily schedule the children were supposed to be participating in table top activities.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One infant did not have an updated feeding sheet.

The operation has not notified parents of the Parents Rights as required by 9/30/23.

Children enrolled on 9/1/23, 9/19/23, 11/1/23 , are missing the emergency contact address on the admission form.
