Based on the information obtained during the investigation inspection, there is enough evidence to show that a child was left alone outside for approximately 1-2 minutes.

Children were observed to be walking around and playing on the play structure holding water bottles.

While reviewing 10 childrens files, it was found that 6 children did not have an emergency contact address. NOTE: This was corrected at inspection when the operation called parents to obtain the information.

A 4 month old child was in a play yard with a play mat. Note: This was corrected during the inspection on 5/4/23 when the child and the play mat were removed from the play yard.

There is sufficient evidence to show that a staff member grabbed a child and picked him up as a disciplinary measure.

The operation did not have Pre-Service or any other training records available for one caregiver.
