Operation had the TV during nap time and there were four children under the age of two years old.

The two children now have health statement in their files.

The consumer product safety recall form was not updated annually. This was corrected during the inspection when the primary caregiver re-certified the form and ensured the home has no recalled items.

Cleaning supplies and other toxic materials were observed where children could have access (restroom, outside and garage).

The primary caregiver didn't inform Licensing that she added indoor space.

The gate leading into the outdoor activity area was not locked.

The substitute caregiver did not have a notarized Licensing Affidavit for Applicants for Employment form available during the inspection.

One of the four children's records evaluated was missing a health statement during the inspection.

The record of attendance for each child in care was not available during the inspection.

The operational policies were missing the following items: 1- Procedures for handling medical emergencies, 2- Safe sleep policy for infants from birth through 12 months old, 3- Promotion of indoor and outdoor physical activity. Your policies must include: A- The duration of physical activity at your home, both indoor and outdoor; B- The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities, C- A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time, 4- Procedures for parents to visit the child-care home any time during your hours of operation to observe their child, program activities, the home, the premises, and equipment without having to secure prior approval.

The primary caregiver was missing the following training: (1)Recognizing and preventing shaken baby syndrome; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS)]; and (3)Understanding early childhood brain development.

Sign-in/out logs were not maintained for 3 months.
