Based on information obtained during the course of the investigation it was determined that this child has left the classroom before on their own. Based on this information care was not consistent with this child's habits.

Based on the information obtained, it was determined a caregiver did not demonstrate good judgement when initiating a conversation with a parent of a child not enrolled in their classroom during class time.

Based on the information obtained during this investigation, there is sufficient evidence to support the allegation of neglectful supervision.

Based on the information obtained, there is sufficient evidence showing a caregiver did not maintain supervision of a three-year-old child as the child was able to leave the operation unsupervised for approximately ten minutes

A three month old was observed sitting at a four seat table. The safety straps were not used.

Personnel records were evaluated for six staff members were reviewed and the following was determined: One staff member needed photo identification. Three staff members needed trainings in all special topics. The director needed transportation training. One staff member needed 11 hours of annual training. Two staff member needed proof of educational qualifications.

The electrical outlet in the infant room are missing outlet covers. The outlet was accessible to the children. Note: The caregiver placed covers in the outlet during the inspection.

A caregiver responsible for 14 children between the ages of 18m-2 years old did not know the number of children they were responsible for.

The infant caregiver did not wash her hands after diapering.

The emergency preparedness plan did not include the following: -How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments. -How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter. -How your staff will evacuate and relocate with the essential documentation including: (A)Parent and emergency contact telephone numbers for each child in care. (B)Authorization for emergency care for each child in care. (C)The child tracking system information for children in care. -How your staff will continue to care for the children until each child has been released. -How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

One caregiver needed training in all special topics.

The caregiver in the infant classroom did not wash the child's hands after diaper changing.
