Toy Box Child Development Center

There were 10 children sitting in bucket seats without the safety straps fastened. This was corrected at inspection once the safety straps were fastened by the caregivers.

A restroom utilized by the children, baseboad was not secured and attached to the wall. This was corrected at inspection once the baseboard was glued and secured to the wall.

Based on a DFPS investigation, there is sufficient evidence of a child leaving the operation building unsupervised.

One classroom contained cleaning supplies and toxic materials in the bottom cabinet accessible to children. This was corrected at inspection once those items were removed and placed in a locked cabinet.

One classroom were missing a total of 8 safety straps for the bucket seats. This was corrected at inspection once the straps were installed.

During a follow up inspection a classroom was out of ratio by 3 children. This was fixed when another caregiver arrived in the classroom. A classroom was out of ratio by 1 when a caregiver left to go answer the front door. This was corrected when the caregiver returned to the classroom.

While reviewing files there was no caregiver qualifications available.

While reviewing records there was no allergy plan available for a child with known allergies.

A slide used by children had cracks with sharp edges present.

While entering the operation a caregiver was heard yelling at children in care.

There was one medication at the operation that was not in the original container and labeled. *This was corrected at inspection when the medication was sent home and not administered to the child.

Out of 10 child files reviewed 5 were missing completed emergency contact information and 2 were missing healthcare information.

On the younger play ground there were two cement blocks that caused a tripping hazard and bolts that were sticking out on the covering. *This was corrected at inspection when the bolts were covered and the cement blocks were removed.

Out of 10 staff files reviewed 8 were missing annual training hours.

Out of 10 staff files reviewed 4 were missing training in CPR/First aid.

Out of 10 child files reviewed 8 were missing health care provider statements.

Out of 10 staff files reviewed 6 were missing proof of education, 2 were missing employment affidavits, 4 were missing a copy of their photo ID, and 2 were missing signed orientation.

There were concerns of the maintenance of the building walls and floors, as follow: *There are holes in the walls with exposed sheet rock. *There are dirty and stained carpets on the floors. *There was a carpet with frayed edges posing a tripping hazard. *There are broken or chipped tiles on the floors. * The walls in the classrooms were scraped causing peeling/chipped paint with exposed sheet rock. There were dirty stains along the walls to the base of the floors.

The infant classroom had 12 children with two caregivers. The maximum group size for this classroom is 10; therefore the class exceeded the group size by 2 children. Note: This was corrected when the oldest two children were moved to another classroom.

Two televisions were not anchored. Note: This was corrected when one television was removed from the classroom and the other television was anchored to a cart.

The bus used for transporting children was not equipped with a child safety alarm.

One caregiver was left alone with ten infants during nap time for approximately fifteen minutes.

Two classrooms lacked a written activity plan.

Five infant feeding instructions have not been updated in the past thirty days.

Two infants were observed sleeping in bouncers during nap time.

During the investigation inspection it was found that the operation did not report a communicable disease to licensing.
