Air conditioning vents and filters in the After school room required cleaning.

The diapering mat in the older infant room had tears which exposed the inner foam, making the mat no longer non-absorbent.

One caregiver hired in May 2024 did not obtain CPR training within 90 days.

1 employee did not have acknowledgment of the required orientation to the operation. The employee acknowledged that she received orientation and this was documented during the inspection.

8 Children's records reviewed did not have notification of parent's rights.

One child enrolled over 1 week did not have a health statement on file.

The operational policies were missing information, including procedures for supporting inclusive services to children with special care needs. This information was added during inspection.

Areas at or near the outdoor playgrounds had old, broken and disgarded equipment that could pose a hazard.

Infant room cribs were not placed far enough apart to prevent infants from reaching into another crib. This was corrected by rearranging the cribs during inspection.

The director and a caregiver have CPR training, but it did not include curriculum that includes use of a CPR manikin and both written and hands-on skill-based instruction, practice, and testing, as required by minimum standards.
