Children were permitted to open a secured door to the day care area during pick up time while outside on the porch. An infant caregiver stopped to talk with other caregivers while taking children outside to the porch, closing the exit door, leaving the children unattended for an unknown length of time.

During the investigation, it was determined that a caregiver props bottles using blankets, rags or boppy pillows while feeding.

The operation failed to inform Child Care Regulation that two infants were left unattended outside within 24 hours of the incident.

Parents were not notified that their children were outside unattended for an undetermined amount of time.

During the investigation, it was determined that a caregiver utilizes blankets with children under the age of 12 months to assist the children in falling asleep by covering their heads.

Two out of five caregivers past their 90 days of hire did not have a first aid certificate on file. Two out of five caregivers had expired first aid certificates from 4/24 and 6/25. Two of the caregivers without first aid training are assigned in the same classroom and have direct supervision of the children.

Five caregiver did not have a CPR certificate on file. Two of the five caregivers are outside of their first 90 days of hire. Two of the five caregivers' certificates expired 4/24 and 6/25. One caregiver was within their first 90 days of hire and a caregiver with a current CPR certificate is present.

A copy of the operation's most current fire inspection was not available for review during the inspection.

A caregiver failed to ensure an infant was strapped in a highchair that contained a five-point harness resulting in the infant being able to lean forward and fall out of the highchair.

Parents were notified of a situation that placed their children at potential risk by being left unattended without adult supervision for approximately 2-3 minutes five days after the incident occurred.

The operation did not report to Child Care Regulation within 48 hours that a classroom of 10 two year old children was left without adult supervision for approximately 2-3 minutes. The report was made five days after the incident occurred.

3 out fo 6 child files did not have a healthcare provider statement on file.

3 out of 6 child files evaluated did not contain an address for the emeregency contact person listed on the child's enrollment form. NOTE: Emergency address information was obtained during the inspection from parents and information was updated in the files.

A current fire inspection was not available for review. The last posted fire inspection was dated 9/14/2020.

The operation did not notify the parent of a situation that place their child at risk until the following day.

Caregivers did not properly count the children in care resulting in a 2 1/2 year old child being left outside on the playground alone.

Caregivers did not provide adequate supervision to the children while on the playground. A child, age 2 1/2 years, was left on the playground unattended for approximately 3 minutes. The child was found by another caregiver and returned to the building unharmed.

Upon entering the building from the playground area, the caregiver did not count the number of children in care to ensure all of the children came inside and assumed all of the children were accounted for at the time. The caregivers did not have a method in place for conducting head counts at the time of the incident. As a result, a child, age 2 years 5 months of age, was left outside unattended on the porch area. NOTE: At the time of the investigation, the caregivers had developed a threshold count sheet system and post the number of children in care during the day to frequently monitor the number of children in their care.
