During investigation inspection, the floors near the sinks in the "Toddler 1" and School-Age 1" classroom were sticky and in need of cleaning. The floors in the "PreK 3 (2)" classroom restrooms were also dirty and in need of cleaning. Old food and used paper items were found on the floor in "Toddler 1" classroom. Unused daily cleaning checklists were found in these classrooms.

The director did not have proof of the 30 hours of required annual training at inspection.

Of the ten staff files that were evaluated, one caregiver did not have a current CPR/First Aid certification. The most recent certification expired August of 2025.

Loose-fill surfacing material on all three playgrounds was measured to be approximately three inches deep. There were small areas that were bare of loose-fill. The inadequate loose-fill depth has left the underlying ground cover exposed which has created a potential tripping hazard for children.

In the toddler classroom, the diaper changing mat was torn. A photo was taken. This was corrected at inspection when the director replaced the changed the mat.

Caregivers with hire date 04/18/2018 do not have the required annual training: -topics for the last annual training year. The -caregivers need training in the following topics: - Preventing and controlling the spread of communicable diseases, including immunizations; - Administering medication, if applicable, - Preventing and responding to emergencies due to food or an allergic reaction; - Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and - Handling, storing, and disposing of hazardous materials.

A review of the Employees Who Left in 2023 data reveal that the operation had not reported the number of employees who left employment in 2023 by January 15,2024, as required.

One infant was observed drinking a bottle while sleep in the crib.

Operational Policies did state if operation will administer unassigned epinephrine auto injectors.

Caregivers with hire dates 02/20/2017 and 08/23/1999 do not have the required annual training topics for the last annual training year. The caregivers need training in the following topics: - Preventing and controlling the spread of communicable diseases, including immunizations; - Administering medication, if applicable, - Preventing and responding to emergencies due to food or an allergic reaction; - Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and - Handling, storing, and disposing of hazardous materials
