Observed two classrooms without a written activity plan.

There is no current fire inspection on file. The last one was done on 8/2/21.

There is a licensed classroom that is currently being used as a storage room.

It has been more than 12 months since the fire extinguishers have been serviced. They were last serviced May 2021.

A fire drill has not been completed since January 2022.

The permit holder did not notify licensing that a new director had been hired.

There were several items that the children have access to when out on the playground. There is a missing gate to the enclosure for the AC unit. The storage building outside is not locked and has items such as paint cans stored inside. There is a large gap of about 9 inches under the storage that is not covered which can allow children to crawl under the bottom. There is a broken pallet that the children have access to. The sign in/out logs are not being properly maintained as thee were only 2 children signed in and there were 13 present in the building. (NOTE: corrected during inspection as the missing children were signed in.)

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Cribs were not labeled with children's names. Note: This was corrected during inspection as cribs labeled.

A fire extinguisher is not secured in the passenger compartment.

Facility does not have a carbon monoxide.

Four cribs in the infant room don't have documentation that each crib meets the applicable rules at Title 16, Code of Federal Regulations, Parts 1219 or 1220.
