Per the DFPS investigation, a caregiver allowed children to incorrectly climb up a slide and jump off an indoor play equipment.

The operation has an indoor play equipment slide that does not meet the use zone minimum of six feet from the perimeter of the equipment requirement.

This standard was reviewed in relation to the reported allegations and found to be deficient. There was evidence that a child was left outside for approximately 2 minutes unsupervised.

Staff at the operation were using diaper rash cream and Vaseline during diaper changing without written authorization from the parents. The Director instructed all staff to remove all diaper rash cream and Vaseline, and to discontinue usage of diaper rash cream and Vaseline until the parents have provided written authorization.

Cots in use did not have a labeling (name to cot) system in place to identify a specific cot to the child that uses them. During the inspection, the staff in each class labeled each cot with numbers and created a list with the numbers and the child's name that will use each cot.

There were sippy cups and bottles in use for children 2 years and younger, that did not have the child's first name and initial of the child's last name on them. The staff added first name and last name to all the sippy cups and bottles.

3 staff's files were reviewed, and the annual training information for the current training year was missing.

The operation electrical outlets where children 4 years and younger can reach were not covered.

Infants in the operation that are receiving formula do not have current feeding instructions.

This standard was found deficient as part of a DFPS Investigation.

A caregiver did not properly account for all of the children in their care when a child that was in their care was left unsupervised on the playground for a lengthy period of time.

The operation has not had a sanitation inspection since 9/30/2022.

In the infant room, there was one caregiver with 5 children with a specified age group of 0-11months. This was corrected at inspection when a second caregiver was moved into the room.

Infant feeding instructions on file have not been updated since 11/2023.

A caregiver with a pending background check was present with children at the time of inspection. This was corrected at inspection when the caregiver was asked to leave.
