A review of the employees who left employment in 2025, revealed that the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

The operation has unpaid background checks fees that have been outstanding since 3/2/2024.

The staff in charge did not have proof of education available during the inspection time.

Director has not corrected the violation of a caregiver not meeting minimum qualifications. This same caregiver has continued to be counted in childcare ratio and was left in charge of the operation during the director's absence.

The operation did not follow the terms of the type of permit that was issued for a Day-care center which is defined by TAC 745.37(2) as " A child-care facility that provides care at a location other than the residence of the director, owner, or operator of the child-care facility for seven or more children under 14 years of age for less than 24 hours a day, but at least two hours a day, three or more days a week. "

The lighting in the infant classroom was not adequate to be able to see the children napping in their play yards.

The operational policies did not include the policiy for promotion of physical acitvity.
