The operation failed to submit the number of employees that have left in 2025.

The operation failed to notify licensing of the move/reconfiguration of the infant room and the toddler room.

It was determined that the operation was not utilizing a piece of outdoor equipment per the manufacturer's instructions or guidelines. The equipment is intended to be a crawling toy instead of a climber.

Two pre-k classroom were noted placing sleeping cots with no space in between each other. Note: Both classrooms arranged the cots with adequate spacing, it was corrected at inspection.

The caregivers in charge of the toddler classroom were negligent when they failed to account for a child who was able to leave the facility unattended and was found 6 minutes later unharmed.

Supervision was compromised when a toddler was left unsupervised and wandered away from the operation. The child was later found unharmed by a bystander.

Based on the video, a caregiver was heard using a loud tone when talking to a seventeen-month-old while the child was still in the crib. NOTE: This caregiver was terminated.

Based on the interviews, it was determined that positive methods of discipline were not used when a caregiver was seen putting a toddler down on his back forcibly in the crib. Based on the video, that same caregiver was seen grabbing a toddler by the forearms out of a crib and placing him on the ground. NOTE: This caregiver was terminated.

Toy bins were observed in several rooms to have an accumulation of dirty on the inside bottom of the bins. The exercisers in the infant room had dirt in the cervices on the tabletop of the equipment. Several toys were found on the playground with cracks that were sharp to the touch. A lattice that is nail to another fencing is broke in several places and is sharp to the touch and accessible to children. This was corrected during the inspection.

Parts of the promotion of Physical Activity was not available for review in the Operational Policies.

One out of eight employees did not have the required hours for face-to-face training.

One child did not have health statements completed by a doctor stating the child was healthy enough to be in daycare after being enrolled in daycare for over a year.

Five out of eight children's records did not have the special needs statement.

The operation did not notify Licensing about a positive COVID cases within the 2 days allowed.

One caregiver was observed in the infant II classroom with direct access to children without receiving an eligibility letter from the background unit. This was corrected when the person in charge removed her from the room.

Three seizure action plans did not have a parent signature or date.

One infant was observed sleeping in a restrictive device.

The stationary tricycles had one broken seat. The wooden border on the playground had an exposed nail accessible to children. A metal school bus was missing caps for the feet which was sharp to the touch.

The antiseptic wipes located in the van were expired. This was CORRECTED when new ones were put into the van.

Two caregivers were observed in the infant room with direct access to children without receiving an eligibility letter from the background unit. This was corrected when the director remove the caregivers from the room.

Caregiver displayed innappropiate methods of discipline front of children.
