The operation applied an over-the-counter ointment on a child without the proper authorization from the parent.

Gas Inspection has not been completed within the two year period. The last gas inspection was conducted on 04/01/2019.

One child's record reviewed did not have the latest immunization record on file available for review.

Operation allowed one caregiver to be present prior to receiving the person's background check.

One infant did not have their monthly feeding schedule updated. The last time the feeding schedule was reviewed was in May 2021.

The Consumer Product Safety Commission annual certification has not been updated. The last verification was 08/01/14.
