The Learning Stampede

A caregiver was observed changing multiple children in the middle of the classroom prior to naptime and did not wash their hands in between changes.

Caregivers used poor judgment when they allowed two children to have access to the kitchen at the operation.

A child was observed sleeping in front of a designated emergency exit door. NOTE: The child was moved during the inspection.

The director had an infant, 5 months of age, sitting in a bouncy seat in the kitchen. The director was relieving a caregiver at the time. NOTE: The infant was removed from the kitchen and returned to the infant room.

A child was observed being sent to the restroom inside from the playground alone and without an adult to supervise them while inside.

The operations playground fence had a loose picket which allowed a child to exit the play area.

Caregivers in two out of three classrooms did not provide the accurate number of children in their care when asked by the Inspector. Caregivers were asked 3 times to ensure the correct headcount number was being provided.

One out of five child immunization records reviewed was missing updated vaccinations.

The fall zone under the climber that measured 24 inches in height, was less than 1 inch deep.

The use zone measured 4 ft from the bottom of the slide to the fence on the large climber.
