A child was observed sleeping on a pillow in side the crib.

A child was observed asleep in a crib with loose fitting sheet.

Children were observed on cots/mats and had been laying there more than 1 hour without being given an alternate activity.

A caregiver was overheard telling children who were awake to hush during the rest period.

A caregiver was observed not washing hands after diapering a child.

Fire extinguishers were not observed in either of the two vehicles.

This is related to a DFPS investigation. Broken floor tiles and dirty carpets were observed in the toddler room and in areas used by the children posing a health and safety risk.

This is related to a DFPS investigation. The operation did not have a record of employment on file for a caregiver. The caregiver was observed providing care to the children.

This is related to a DFPS investigation. A child was prohibited from active play when the child was placed in a child-sized highchair in the room away from the other children who were engaged in active play.

During the inspection a 2 month old was observed to be in the pre k classroom. All 10 of the other children present were greater than 18 months older than the infant.

The operation stated that for discipline the children are given a time out for "about 10 minutes".

During the monitoring inspection it was found that two of the caregivers who provide transportation did not have their required two hours of transportation training.

During the monitoring inspection it was observed that there were 4 rooms of napping children with no caregivers present in any of the rooms. There were 20 children left unsupervised ranging in age from 12 months - 4 years old.

During the monitoring inspection it was observed that a caregiver changed a child's diaper and did not have them wash their hands afterwards.

During the monitoring inspection a caregiver began serving snack to the children without washing their hands first.

During the monitoring inspection it was observed that the bathroom in the pre k classroom was not equipped with soap for hand washing.

During the monitoring inspection a caregiver stated that due to the sink in the infant room not working, infants use a little bit of hand sanitizer to keep their hands clean.

During the monitoring inspection it was observed that the first aid kit in the transportation vehicle was missing tweezers and a thermometer.

During the monitoring inspection 2/4 of the rooms observed during naptime did not provide enough lighting for adequate supervision of the children.

During the monitoring inspection it was observed that the van used to transport 5 children, four of them between the ages of 4-6 years old, was not equipt with any booster seats or forward facing child safety seat.

During an inspection it was found that caregivers were hitting children as a way to control their behaviors.

During the follow-up inspection the pack n plays were observed to have sheets on them that presented an entanglement hazard and were not snug fitted.

During the follow-up inspection emergency drill form was reviewed and the last documented drill was 12/2022.

During the follow-up inspection the operation was re-cited for not having a current activity plan posted for all age groups. There was not a daily schedule posted for pre k or toddler aged children.

During the monitoring inspection cleaning supplies and Raid were observed in the toddler bathroom, under the sink, without a lock and hand sanitizer and cleaner were observed in the pre k room on a low shelf, within reach of the children in care.

During the monitoring inspection the pack n plays were observed to have sheets on them that presented an entanglement hazard as they were not designed for the pack n plays they were being used on.

During the monitoring inspection, screen time was observed and it did not meet educational goals. The children were observed watching Sonic.

During the monitoring inspection the fridge in the infant classroom was observed to have thawed breast milk with a date from June marked on it, as well as the fridge having hair, and dirt or old food stuck to it, needing to be cleaned out.

During the monitoring inspection the infant's pack n play and sheets were observed to be dirty, with dirt, spit up and food crumbs. The floors throughout were observed to have hair, dirt and crumbs. The toys in every classroom were observed to have dirt on them. The trash cans and stools in and around the operation were also observed to have dirt and other things stuck to them.

During the inspection it was observed that children were eating breakfast without washing their hands first.

During the monitoring inspection it was observed that there was a shelf that was hung in the infant room, over pack n plays, that was coming out of the wall and a falling hazard. There were also two storage cabinets in the bathrooms that were a falling hazard and needed to be secured to the walls. On the playground the tarp was also coming up from under the loose fill, presenting a potential tripping hazard.

During the monitoring inspection it was found that the operation did not have a current activity plan posted for all age groups. There was not a daily schedule posted for pre k or toddler aged children.

During the monitoring inspection emergency drills could not be reviewed as there was not an emergency drill form. One was emailed to the operaiton to begin using.

After reviewing child/caregiver ratios, it was determined there was more than an 18 month age difference between the youngest infant and the oldest child in a group. The youngest child was eight months old and the oldest child was 3-years-old. This was corrected at inspection when the oldest child was brought to a different classroom, bringing the facility into compliance.

The carbon monoxide detector had not been tested monthly. The carbon monoxide detector was tested during the inspection, bringing the facility into compliance.

After a review of children's records, three children enrolled in care did not have immunization records on file for review.

A feeding utensil (sippy cup) was observed sitting on the handwashing sink in the infant classroom. The sippy cup was immediately removed when addressed and this deficiency was corrected at inspection.

A toy box on the playground was observed to have shear points on it.

The First Aid kit was found to be incomplete. There were no scissors nor tweezers.

Ceiling tiles in the infant and pre-k/school-age classrooms were separating from the ceiling. Paint was peeling from the wall in the 2-3 year-old classroom and in the pre-k/school-age classroom. The baseboards were separating or missing from the wall in the 2-3 year-old classroom. A portion of a wall in the 2-3 year-old classroom needed to be cleaned as stains were visible.

A rug in the toddler classroom needed to be cleaned as stains were visible. Sinks in the toddler and infant classrooms needed to be cleaned as stains were visible. Both sinks were immediately cleaned when addressed, bringing the facility into compliance regarding the sinks.
