Two caregivers did not know the number of children they were responsible for when asked twice. This was corrected when the caregivers counted the number of children and gave the correct number.

An infant under the age of 12 months was observed asleep in a play yard with a pacifier string attached to their clothing. The pacifier string was removed during inspection.

An infant under the age of 12 months was observed asleep in a play yard with a loose-fitting sheet. The infant was removed from the play yard at inspection.

An infant under the age of 12 months was observed asleep in a play yard with extra bedding. The manufacture instructions of the extra bedding stated, "Never use in a crib or play yard". The extra bedding was removed at inspection.

While reviewing video footage for another investigation, a caregiver was not observed washing any of the infant?s hand throughout the day. The caregiver is only observed wiping the children?s hands and faces with wipes.

While reviewing video footage for another investigation, a caregiver was not observed washing her own hands between each diaper change.

While reviewing video footage for another investigation, a caregiver was not observed washing her own hands before preparing meals.

While reviewing video footage for another investigation, a caregiver was observed preparing an infant?s food using a utensil to try the food and then using the same utensil to prepare the food on the plate. The caregiver was observed eating food off of a plate and then giving the infant the remainder of the food on the plate while using the same utensil the caregiver ate off of.

While reviewing video footage for another investigation, a caregiver was observed not disinfecting the diaper changing mat between each use.

While reviewing video footage for another investigation, a caregiver is observed exiting the classroom on multiple occasions in short durations while the infants are eating their food.

While reviewing video footage for another investigation, a caregiver was observed using her cellphone on multiple occasions while caring for the infants in the classroom.

While reviewing video footage for another investigation, a caregiver used bad judgement when attempting to move the diaper changing mat while an infant is on it. The caregiver is observed sitting on the floor with the diaper changing mat in front of them, picking the diaper changing mat up while an infant is on it and dropping it. The diaper changing mat was padded and landed up correctly however, the caregiver exhibited bad judgement when attempting to move it this way.

While reviewing video footage for another investigation, an infant was observed sleeping in an exerSaucer and an infant was observed sleeping in a bouncer.

While reviewing video footage for another investigation, a caregiver was observed inappropriately grabbing children by their arm when picking them up and moving them around the classroom.

While caring for infants a caregiver was observed washing dishes, sweeping, vacuuming and spraying disinfectant in the room.

The operation did not notify parents within the required 2 days when their child was placed at risk.

During a DFPS investigation, there was sufficient evidence to support that parents are only allowed in the doorway of the operation, they are not allowed in the classrooms.

During a DPFS investigation, a caregiver admitted to smoking a THC vape pen in the infant classroom multiple times over multiple days. The vape pen was used in the classroom around the infant as a result an infant tested positive for cannabis.

Infants are being placed in a restrictive device as a form of discipline.

During a DFPS investigation, there was sufficient evidence to support that the director was not appropriately supervising caregivers when a caregiver was observed on video walking out of a classroom leaving infants unsupervised multiple times.

During a DFPS investigation, there was sufficient evidence to support that a caregiver did not demonstrate good judgement when the caregiver sprayed Lysol in the close proximity of infants. During a DFPS investigation, there was sufficient evidence to support that a caregiver did not use good judgement when they used their sleeve to wipe an infant's nose twice. Additionally, the caregiver did not wash hands after doing so.

Two infants did not have infant feeding instructions available for review.

One infant was sitting in a highchair without the safety straps fastened. This was corrected when the caregiver fastened the safety straps.

The stationary equipment outside did not meet the required use zones. There was a wooden balance beam in the use zone of the ladder of the stationary equipment.

During the walk through of the infant room, a feeding plate was observed in the sink used for hand washing. This was corrected when the caregiver removed the plate from the sink.
