Two children did not have a health statement on file.

One caregiver did not have a Pre-Employment Affidavit for Applicants for Employment on file.

One caregiver did not have a Notarized Affidavit for Applicants for Employment on file.

It was determined that a caregiver demonstrated poor judgement by not intervening with inappropriate and/or unsafe behaviors when redirection proved ineffective.

It was determined that a caregiver forcefully grabbed a child by the arms, causing bruising on the child's right and left arms.

Based on the findings of a DFPS investigation, it was determined that the director did not demonstrate competency and good judgment when failing to follow procedures which resulted in release of the wrong child. The child was promptly returned to the center. (Note: The director is no longer employed).

Based on the findings of a DFPS investigation, it was determined that the director did not follow the center's policies with regard to releasing a child to someone other than the parent. The director did not make a photocopy of the identification presented at the time of release as required by the operation's procedure. As a result, the Human Services Technician mistakenly transported the wrong child from the operation to the child's home. Upon realizing the error, the Human Services Technician immediately returned the child to the center. (Note: The director is no longer employed).

Based on the findings of a DFPS Investigation, it was determined that the director did not ensure the operation was operating in compliance with the minimum standards. The director knowingly provided false information and withheld pertinent details from the child's parents regarding an incident that placed their child at risk. The parents were told that their child never left the premises of the center. (Note: The director is no longer employed).

Based on the findings of a DFPS investigation, it was determined that the director interfered with an investigation by knowingly providing false information when reporting an incident to Child Care Regulation. The director reported that the child did not leave the premises, but the director was actually aware that the child left the premises and was transported away from the center by the Human Services Technician. (Note: The director is no longer employed).

Two caregivers did not have current CPR/first aid training available for review at the time of inspection.

One staff file lacked the pre-employment affidavit for applicants form.

One staff file lacked a copy of the employee's I.D.

A background check was not submitted for one employee. The employee was present at the operation and had direct access to children in care. (Note: The employee left during the inspection and a background check was submitted).

It was determined that caregivers did not conduct an accurate name to face accounting of children and did not maintain knowledge of the number of children they were responsible for. This resulted in a child being left unsupervised.

It was determined that two caregivers left a child unsupervised in the classroom, during the transition to outdoor play time. The child was discovered in the classroom by another caregiver.

Based on information obtained, it was determined that the director influenced staff?s written statements by having staff re-write their statements and telling staff what to write on their statements which misrepresented the facts regarding the incident.

It was determined that the caregiver that administered first aid treatment to the child did not have a current certificate of training in pediatric first aid.

Two children's records lacked a health statement.

Operational policies provided for review did not contain all the required policies and will need to be updated to include the missing policies.

One personnel file lacked a copy of the employee's photo ID.

One personnel file lacked a notarized affidavit.

A copy of the most recent health inspection was not available for review at the time of inspection.
