Four children files are missing information in the special care statements.

Three staff files were missing a signed and dated statement that they have recieved the center policies.

Three staff are missing the following trainings; Recognizing and preventing shaken baby syndrome and abusive head trauma, Understanding and using safe sleep practices and preventing sudden infant death syndrome and Understanding early childhood brain development.

Staff files were observed missing the following trainings for the last full year; -Two staff are missing training in preventing and controlling the spread of communicable diseases, -Five staff are missing training in preventing and responding to emergencies due to food or an allergic reaction, -Three staff are missing training in understanding building and physical premises safety, and -Three staff are missing training in handling, storing, and disposing of hazardous materials

One child does not have the epiPen on file that is listed on the food action plan.

During the inspection, it was determined that the Infant A classroom has not been going outside.

The menus observed for Weeks 1 through 4 do not follow the USDA food guidelines.

Infants and toddlers were observed eating entire pieces of broccoli for lunch. The broccoli was not cut up.

During the inspection, a caregiver did not wash their hands after diapering two children.

Two children did not wash their hands after having diapers changed.

Based on interviews during the investigation, it was determined that infant handwashing is not properly being done.

Ten children's files were observed and missing the updated required information for special care statements.

One caregiver during the inspection, did not know the correct ages of the children that were in the classroom.

One staff does not have a valid certificate for first aid.

Seven feeding instructions have not been properly updated.

Ten children files are missing the new section for if the children can swim and if not, what assistance is needed.

One staff does not have a valid certificate for CPR.

In the two infant classrooms, the cribs were touching next to each other, while the infants were napping.

It was determined that children are not recieving the same napmat everyday. Napmats in some classrooms have numbers and some do not. Some classrooms have number charts which are not being followed or have not been updated.

The operation did not notify Child Care Regulations within 2 days of receiving notification of positive cases.

The Infant A classroom was observed out of ratio. There was one caregiver and 6 infants with a specified age group of 0 to 11 months.
