The Director did not instruct staff (written or verbal) to adhere to Child Care Regulation?s policy, concerning the immunization documentation for a child at the operation.

A child's file was missing immunization documentation.

Based on the DFPS investigation, it has been determined that caregivers have exposed children to prohibited punishment.

A caregiver failed to immediately provide CPR to unresponsive child.

A caregiver was responsible for an ill child that needed immediate medical attention, and did not contact emergency medical services. Instead contacted a relative of the caregiver.

A caregiver was caring for an ill child who needed immediate medical treatment, and the caregiver did not contact emergency medical services immediately.

A caregiver was caring for an ill child and did not have current first aid certification.

A caregiver was caring for an ill child and did not have current cpr certification.

A child was left unsupervised in a classroom at the end of the day.

A caregiver was unaware of a child being left in her care and as a result, the child was left unsupervised inside the classroom.

The operation failed to notify Child Care Regulation about 2 additional rooms opened for childcare without prior approval.

One out of eight staff filed did not have a notarized affidavit for review during the visit.

The operation failed to report the number of employees left for year 2023 by January 16, 2024.

Based on the gatherings during the investigation, it was determined the operation failed to contact Childcare Licensing within 2 days. It was a total of 15 days before the notification took place.

The fire inspection was last conducted in 9/11/23 and corrections were to be by 9/25/23. There were no inspection that showed the corrections available during the visit.

The recall product document has never been completed. No completed document was available for review during the visit.

A staff was in the 2-3 year old class with direct access to 14 children who were in care with a pending background check status.

Based on the information gathered during the investigation, it has been determined that a child was unattended, and caregiver was not at a physical proximity that allowed visual and/or auditory awareness of the child in care.

Based on the gatherings during the investigation, it was determined that the operation failed to notify Childcare Regulations of a child placed at risk within two days.

Six caregiver files did not have CPR/First Aid cards on files.

One infant was swaddled and does not have a sleep exemption form on file.

The operation did not have a copy of the CPSC form. This was corrected at inspection when the operation signed up for CPSC reports, printed, and posted the form.

Water is offered as an option during meal and snack times.

Infant feeding instructions for one infant had not been updated for more than 30 days.
