Based on interviews conducted, one caregiver left another caregiver alone with 17 three-year-olds for more than ten minutes.

Based on information received, a child was left unsupervised in a hallway for 2 to 3 minutes.

Based on sign in and out sheets two classrooms were out of ratio in the month of August. One classroom had one teacher to five infants. The second classroom had twenty 3-year-olds to school age children with one teacher.

Carbon Monoxide detectors were last checked on 4/23/2025.

Lockdown drill was conducted once on 3/21/2025.

Due to the amount of deficiencies cited. Director has not ensured operation is maintaining compliance with minimum standards rules.

Pediatric First Aid training expired for one caregiver on 7/2025.

Smoke Detectors were last checked on 4/23/2025.

Operational Policies did not have parents rights in handbook.

Fire Extinguishers were last checked on 4/23/2025.

Fire Inspection was last conducted on 6/18/2024.

During walk through, one out of the four classrooms observed had three caregivers with 11 small children.

During follow up inspection, one classroom was observed out of ratio. Classroom had three teachers with 11 infants. The specified age group was 0-11 months, making them one child over the requirement.

During inspection, two classrooms were observed out of ratio. One classroom had three teachers with 12 infants and the second classroom had three teachers with 19 toddlers.

During follow up inspection, a fire inspection had not been conducted for 2024. Previous Fire Inspection report is dated for 4/28/2023.

Based on information from DFPS, it was found that prohibited punishment was used when a child was slapped on their face by the cargiver.

During investigation inspection, operation did not report change in director. Operation changed director on 7/5/2023.

During investigation inspection, licensing incident report form was not used for child who required medical attention.

Staff hired on 7/11/2023 was observed supervising children without and eligble background check result.

Operation did not notify licensing of child requiring medical attention as required.

Operational Policies did not discuss if the operation will administer epinephrine auto-injectors.

Staff hired on 6/20/2022 had online CPR training.

Staff hired on 8/31/2022 had 8 hours of annual training for 2021-2022 year.

Staff was mopping the floor while children were in care.

Seconds of food is not being served consistently to children.

The operations menu and food served does not follow the meal patterns established by the Child and Adult Care Food Program (CACFP)

The fire drill log was not documented for the month of September. This was corrected at inspection when the drill log was updated with the date the drill was conducted.

One child was missing the health statement.

One child, aged 4 years did not have a hearing and vision on file.

Three infants feeding sheets had not been updated with the parent signature.
