Camera footage reviewed showed a caregiver aggressively putting a pacifier in a crying infants mouth, aggressively grabbing the infant's arm, and aggressively patting them on the back.

Two infants were observed sleeping in a swing. The director moved the infants from the swings and placed them in their cribs.

During the walk-through one caregiver was observed to be supervising six infants.

The drills logs reviewed did not contain the time the drill occurred.

One of the centers transportation vehicles did not have a first aid kit.

One of the centers vehicles did not have an electronic safety alarm.

A caregiver did not use good judgment or self-control when she aggressively handled a child who was exhibiting difficult behaviors.

There was a diaper changing mat in the infant room that had a tear. The person in charge corrected this at the time of the inspection by replacing the mat.

There were two trash cans without lids that contained food. The Director corrected this at the time of the inspection by replacing the trash cans.

During the inspection a caregiver was caring for five children ages-6 months, 9 months, 10 months, 12months, and 15 months. This made the specified age group at 10 months causing the caregiver to be out of ratio. The ratios for this age group are 1 teacher per 4 children. This was corrected at the time of the inspection when another caregiver entered the classroom.

A caregiver's purse was accessible to children. The caregiver corrected this at the time of the inspection by moving the purse to a location that was not accessible to children.

The Director that he admitted that he used raid on ant beds on the playground, he not a licensed pest exterminator.

A teacher was in the one-year-old room caring for 4 children. When asked how many children she had she said 5. She had an infant in the infant room sleeping in a crib alone with no caregiver in the room. She said she thought both rooms were one room. This was corrected at the time of the inspection when the child was moved to the one-year-old room and another caregiver came to help in the room.

Based on the information gathered during the inspection through interviews and viewing the video footage it was found that children were left unsupervised in the infant room, the one year old room and the two year old room on several occasions for unknown amounts of time.

During the inspection and also viewing the video footage it was found that the caregivers were not using the safety straps in the seats at the table nor on the highchairs.

Based on the information obtained there was enough information obtained that the playground was not being inspected daily for any hazards.

The Director was not keeping playground maintenance records.

Based on the information obtained through interviews it was found that children were not receiving diaper changes in a prompt manner.

The Director was no ensuring that caregivers were following the Minimum Standards with supervision, proper diapering procedures, and safety practices.

Based on the information obtained during interviews, caregivers were using diapering products that did not belong to the child on children without the parent's written permission.

Based on the information gathered during the inspection through interviews and viewing the video footage it was found that children were left unsupervised in the infant room, the one year old room and the two year old room on several occasions for unknown amounts of time.

There were diaper rash creams and two other medications in children's cubbies that were not labeled with the child's name.

Two rooms were dark enough that Licensing had to adjust eyes to see when entering the room. This was corrected during the inspeciton when the caregivers turned on lamps.

Based on the information gathered there was enough evidence gathered that caregivers were using prohibited punishments.

Based on the information gathered it was found that infants were not having outdoor play opportunities.
