There is a medication without a child name and it is unknown who it is for.

Two children with food allergies, have expired epiPens on file at the center.

Four staff files were observed missing the following training hours for the last full year; (1)Emergency preparedness (2)Preventing and controlling the spread of communicable diseases, (3)Administering medication, (4)Preventing and responding to emergencies due to food or an allergic reaction, (5)Understanding building and physical premises safety, (6)Handling, storing, and disposing of hazardous materials

Ten childrens files were observed and the following is missing; Two children files are missing health care information, One childs file is missing a special care statement, Five children files are missing the parents received the operational policies, One child is missing a health statement. and Three children are missing hearing and vision.

Three children with food allergies were missing the action plans and the medication.

Three caregivers did not know the how many children they were supervising. Two caregivers did not know the ages of the children that they were caring for.

The caregiver did not realize that a child was missing from the classroom. One caregiver was not supervising two young children who were in the bathroom at the same time.

A caregiver did not wash a child's hands after a diaper change.
