It was observed by Child Care Regulation a teacher spoke to a child in an aggressive manner by stating "You do not touch that," The child started crying.

The caregiver who transitioned to take over group 2 was unaware of the number of children she was caring for. The caregiver utilized the incorrect checklist for the classroom in her care which was 25. The incorrect checklist was removed from the binder and updated checklist was discovered on the next page.

When staff was asked how many children she had. The teacher had to count how many children were present.

Two four year olds were mixed in with after schoolers on the bus. The age limit for the center is 5 years and older.

The policies do not address the promotion of indoor and outdoor physical activity.

There is a cracked light in the galaxy room, two cracked light fixtures in the game room, and water mark in the art room. Note: Pictures were taken. The first aid kit on bus one had expired supplies such as brown gloves, expired ointment and gauze that was brown too. Note this was corrected during the inspection when the items were replaced.

The children were watching the movie Home. It is not educational.

One of the sinks in the boy's restroom does not work. A picture was taken.

A child does not have a food allergy plan.

Two children had expired medication.

A child's food allergy plan is not posted.

The emergency preparedness plan does not address contunity of care.

Based on a DFPS investigation a caregiver did not supervise children as they were sleeping while they were supposed to be supervising children during movie time. The caregiver also did not properly supervise the children when they were allowed to crawl under the stalls of the restroom. These instances of a lack in supervision is what resulted in a child being inappropriately touched.

Based on the recent DFPS investigation this is the third time the operation has been cited for abuse/neglect of children. The director has failed to uphold director responsibilities and ensure the ongoing compliance of the operation.

Based on a DFPS investigation there is reason to believe neglectful supervision occurred when a child was touched in an inappropriate manner.

Two stalls in the girls restroom did not have toilet paper. This was corrected during the inpsection when the toilet rolls were placed in the toilets.

Chipped paint is in the napping room. Pictures were taken.

Staff did not know the ages of the children when asked. This was the art room.

Children were not signed in. This was corrected during the inspection when the children were signed in. This was the kinder class.

Based on a DFPS investigation an employee abused a child when she slammed the child on their napping mat on purpose, did not rendered aid immediately, and resulted in a red mark being left on the child's face.

Based on a DFPS investigation children were forced to sleep for naptime. One child was forcefully placed on a mat to sleep and another child had toys taken from them in order to make them sleep.

Based on a DFPS investigation corporal punishment was used on a child when the child did not want to take a nap. The caregiver slammed the child onto their mat as a form of punishment.

Based on a DFPS investigation an employee did not show good judgement or self-control when she dropped a child on their mat in forceful manor.

Based on the information gathered management did not show good judgment when concerns about a caregiver were expressed. A parent expressed concerns not letting their children sit in caregiver's laps. Management did not verify whether or not a caregiver requested permission from parents to interact with children outside of the operation. Follow-up inspection completed on 03/20/23 staff put in the polices staff cannot have contact with children via social media. Staff will have a conference with parents and come up with a solution when concerns arise.

Based on the information gathered staff carried their cell phone while caring for children.

Based on the information obtained a caregiver was engaging in behavior with a child inappropriately while other children in the group were not supervised appropriately.

The provider has several ceiling tiles with water marks. They are located in the following areas: art room, drama center, and one by exit door and the restrooms. There is chipped paint in the kidsercise room. This was corrected during the inspection.
