Three fire extinguishers were sitting on the floor in the kitchen.

A large amount of gnats were observed in the kitchen area and bathroom areas. Furthermore, the operation was observed to be cluttered and not prepared to receive children when they opened for the day. Rooms had cleaning supplies and other supplies in piles and accessible to children.

Caregivers were missing required training certificates.

Staff training records were not available to review.

Evidence gathered during this investigation shows the operation did not report the incident within the required time frame.

There is broken equipment and debris on the playground that needs to be removed. There are boxes, building supplies, and other materials in some of the classrooms that need to be removed.

Several of the children's records reviewed were missing health statements, emergency contact information, or immunization records.

There is a shed door on the playground that will not close properly. There are cleaning supplies stored within reach of the children in the school age building. These pose a hazard to the children.

Staff training records were not available to review.

Two of the vehicles used for transportation are missing a fire extinguisher and a first aid kit.

Several of the kids records were missing items including medical authorization consent.

Evidence gathered during this investigation shows children were allowed to use their tablets and phones while in care. The children were viewing information that was not age appropriate or part of the operation's planned activities for educations goals.

During the investigation inspection, the playground was observed to have broken bricks, wire, and broken toys on it. Also the cleaning supplies in the school age room needs to be moved out of the children's reach.

During the unannounced inspection, there were children in separate classrooms with only one caregiver. Children were left unattended as the caregiver left one classroom and went to another classroom where other children were playing. During the inspection, another caregiver arrived and went into the classroom correcting the deficiency.
