There is no current fire inspection. The last one was completed on 1/10/2025.

For the year being reviewed, staff files reviewed were lacking the following training topics: preventing and responding to emergencies due to food or an allergic reaction and handling, storing and disposing of hazardous material.

There was one caregiver with 7 infants. (NOTE: Corrected during inspection as the caregiver who was out to lunch returned to the classroom)

The posted activity plan in the toddler room was not dated.

The diaper changing mat in the infant room had a covering which made the mat absorbant and it could not be easily cleaned.

There was an infant asleep on a mat whose face was covered by a blanket. (NOTE: Corrected during inspection as the caregiver removed the blanket from infants face.)

There was no completed set of operational policies available for review.

There were containers of LYSOL/Clorox wipes sitting on a shelf that were accessible to children.

Training hours were missing in two records reviewed.

First aid kit was misisng on trasportation vehicle.

Child was observed sleeping in a torn nap mat. Note: This was corrected during inspection as nap mat was removed.

Trash cans in restrooms were observed without a lid leaving toilet paper accessible to children.

Children's information was missing on transportation vehicle.

A 3 years old child was observed sleeping in a playpen during naptime.

Fire extinguisher was missing on transportation vehicle. Note: This was corrected during inspection as a fire extinguisher was placed on the vehicle.

Mos recent health inspection was not available for review.

Classroom was observed over ratio as one caregiver was supervising five infants 8 - 13 months and two toddlers 19months and 2 year old. Note: This was corrected during inspection as owner stepped into the classroom and a second caregiver came in to the classroom.

One out of seven records reviewed was missing immunization records.

Director does not have annual training hours for cycle 6/19/19 to 6/18/20.

A staff file is missing employment affidavit.

Three staff members are not current with transportation training.

A fire drill has not been conducted.

Three employees are not current with CPR training.

Two staff members are missing the following training: Recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices and preventing sudden infant death syndrome(SIDS) and understanding early childhood brain development.
