Four out of ten files did not have all the completed training available for review.

The standard was evaluated for a standard investigation and was not found compliant. Based on interviews with staff, it was determined that a young child was left alone unsupervised in a classroom; the young child was found by their parent.

The operational policies did not list transportation policy and procedures.

The caregivers that transport children during field trips do not have current transportation training-training expired May 25th and May 26 2021.

The operation did not report a suspected case of abuse within the required 48-hour time frame as specified by Texas Family Code 261.101(b). The operation was made aware of the suspected case of abuse on 7/1/21 but did not make the report until 7/5/21.

A caregiver was present in a classroom without having an active status for their background check. This was corrected at inspection when the caregiver left the operation.

A medication for one child was past the expiration date. This was corrected at inspection when the parent was notified to pick up the expired medication.
