At the time of the inspection the operation had not had a gas inspection completed in the required time frame.

A caregiver sprayed a child with soap and water while cleaning the tables.

This was evaluated as part of a DFPS investigation and found deficient. Based on information provided the operation did not create an incident report for an incident where a child was found in a closet inside of a classroom.

This was evaluated as part of a DFPS investigation. Based on information provided it was found that a child was in a dark closet space inside the classroom while the caregiver was changing diapers. The operation director and staff were aware of this happening and made no reports.

This was evaluated as part of a DFPS investigation and found deficient. Based on information provided a caregiver was assigned to a classroom where multiple employees stated the caregiver had difficulty managing.

At follow up it was discussed that caregivers are able to use a child lock on the closet doors in the classrooms and it was discussed with caregivers that these should be closed so children do not enter.

At the time of the inspection the border of the school age playground had bolts standing up from the border that could catch on a shoe or clothing item causing a fall. There is a section of the playground border that has a crack that creates a concern for pinching or a child getting their finger stuck in this.

At the time of the inspection the loose fill around the preschool playground structure was not maintained at a depth of six inches. Dirt underneath fill was visible without effort of moving it and grass growth through the fill was visible.

At the time of the inspection it was found that the use zone for 2 slides completely overlapped one another.

One structure had a rope web that children could climb on, but no clear separation of the structures for the younger and older age groups. The structure with the flexible web rope climbing piece would be accessible to children under 5 years of age.

At the time of the inspection all bathrooms present had a push button lock in each classroom. The door handle had no exterior mechanism for the door to be unlocked.

At the time of the inspection one playground for children younger than 3 years had no loose fill and the other 2 playgrounds did not have the required 6 inches of loose fill.

At the time of the inspection there was not a display for the area for required postings.

At the time of the inspection a box cutter, metal pole, and metal ribbon of loose measuring tape were found on the playground area. In the infant classrooms two pull up bars were mounted with screws missing and the bar being loose and wobbly.
