Child was denied food as a form of punishment.

The permit holder has failed to ensure compliance with the minimum standards by failing to report a child was left unsupervised outside with non-staff members after being notified by the parent of their concerns.

Infant room had 11 infants and 2 caregiver in the classroom. The specified age of the children in the group was 0-11 months, which only allows a maximum of 10 children, two or more caregivers may supervise.

Director failed to operate in compliance of the minimum standards, by allowing caregivers to have outside time in the front of the operation's parking lot.

Operation did not have a fenced in outdoor activity space in front of the operation.

Operation allowed child to be present at the operation without a completed food allergy emergency plan signed by the heath care professional and parent.

Operation allowed a 10 year old child to be present around infants in the 12 -18 month old classroom. 10 year old was more than 18 months older than the youngest child in the room.

An infant (3m) under the age of 12 months was observed with a burp cloth in the crib placed over the child's neck.

Director did not ensure that employees complied with the minimum standards in the infant room on multiple occasions.

Caregiver with a provisional eligibility which restricts them from being alone with children, was in violation when they were left alone in a room with 4 infants.

Infant classroom with a specified age 12- 18 group was out of group size. It was observed 14 children with 2 caregivers.

4 infants were observed sleeping with blankets covering their heads.

The permit holder has failed to ensure compliance with the minimum standards, as demonstrated by 9 high deficiencies and 7 medium high deficiencies within last 24 months which included several infant safe sleep deficiency.

A caregiver was observed with no access to a walkway without having to walk on or over the cots or mats of other children.

Director did not ensure that employees complied with the minimum standards in the infant room on multiple occasions.

The infant classroom exceeded group size. The classroom had 12 infants with 2 caregivers. Of the 12 infants, 8 were 11 months or younger. The specified age of the children in the group was 0-11 which has a maximum group size of 10 infants to 2 caregivers.

5 infants under the age of 12 months were observed with blankets in the crib.

Permit holder failed to comply with law and minimum standards by not having a procedure in place to allow the director to have access to records when the governing body designee is not present at the operation.

The operation did not notify the parent immediately upon an infant falling off a diaper changing table approximately three to four feet to the ground and onto a hard surface.

Operation failed to provide access to children's record during an investigation conducted by DFPS investigator.

A caregiver did not use safety straps located on the diaper changing table, bent over, and removed her hand from the infant while on the changing table. The infant fell off the table and fell approximately 3 feet to the ground.

During a DFPS investigation a caregiver was found Reason to Believe for neglect of a child.

9 month old infant was left in a crib with a blanket and stuff animal.

Infants were unsupervised in a separate room designated for sleep without caregivers being present in the room.
