During the monitoring inspection it was observed that there was an outdoor play equipment structure in the outdoor area that did not have proper loose fill installed under it.

During the inspection it was found that the operation was not documenting the emergency drills that they were practicing. This was corrected at inspection when a drill was conducted while I was present and it was documented accurately.

During the monitoring inspection it was observed that the most recent licensing inspection was not posted. The last one posted was from 2021. There was an inspection in 2022 that the posting should have reflected.

During the inspection it was observed that the children were served snack before washing their hands.

During the monitoring inspection it was observed that there were 42 children and two caregivers in the gym of the operation. This was over maximum group size and ratios by 7 children.

During the monitoring inspection the vehicle's first aid kits were found to be missing some of the required items. This was corrected at inspection when the missing items were replaced during the inspection.

During the inspection it was found that there was a child who attends the operation and has an epi pen for food allergies but the child did not have a specific food allergy plan as required by state regulations.

During the monitoring inspection it was observed that two of the six transportation vehicles used did not have an electronic child safety alarm installed and one of the vehicles did not have it installed properly.
