Upon arrival the operation was out of ratio with one caregiver to four infants and 6 toddler, preK school age children.

Emergency medical authorization was not signed for a child enrolled.

Director in place did not change roll for background check submitted for a volunteer that changed to staff. This standard has been corrected. I have discussed with the employee submission of background checks and the employee submitting background checks understands to submit the email address of the subject. The employee's role has been changed in her background check.

During the investigation it was found that two children were left unattended in a transportation vehicle.

The director did not use good judgment when dealing with a potential exposure to COVID, by leaving children alone in a transportation vehicle.

During investigation it was found that the operation did not report a situation that placed children at risk. Two children were left unsupervised in a transportation vehicle.

During the investigation it was found that the director was found reason to believe for neglectful supervision when she left children unattended in the transportation vehicle after school pickup.

Employee with a volunteer background check was observed alone with children. Volunteer background check does not allow a care giver to be alone with children or counted in ratio.

During an investigation it was found that the parent was not notified when children were left in the transportation vehicle alone in the center parking lot.

A child was sitting in a table seat without the straps being fastened. This was corrected during the inspection when brought to the caregiver's attention and she fastened the strap.

Caregiver did not wash her hands before serving children snack. Handwashing was discussed with the caregiver.

Based on interviews it was found that the caregiver inappropriately grabbed a child by the arms.

Based on interviews it was found that the caregiver was yelling at a child.

Based on interviews it was found that the caregiver did not demonstrate self control in the presence of children when making several inappropriate gestures to child in care.
