Based on the findings from the investigation, it is concluded that a caregiver failed to actively supervise the children while they were using the restroom and did not intervene to ensure their safety, including that of the victim.

Verification of the monthly inspection of the active play space and equipment was not available during the inspection.

During the investigation conducted by the Department of Family and Protective Services, Physical Abuse was found at the operation.

During the playground inspection, it was discovered that the water play table was filled with water and leaves. In response, the caregiver took immediate action and proceeded to drain the water table, effectively resolving the issue. Pictures were taken.

One caregiver is currently pending First Aid and CPR training

The emergency preparedness plan was missing a statement regarding 1)How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments 2) How your staff will continue to care for the children until each child has been released, and 3)How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

There were not enough available toys in the toddler class during a child led activity. Children were observed fighting over the limited amount of toys of available to them during the activity.This was observed through video obtained during the course of the investigation.

A caregiver did not use positive methods of discipline and guidance when redirecting a child in care. The caregiver uses physical redirection to deter a child from a negative behavior and to direct them back to the play area.

During the investigation conducted by the Department of Family and Protective Services, Physical Abuse was found at the operation.
