Numerous children's records were found to be missing the Health Statements in their admission forms.

This standard was evaluated as part of an investigation and found to be deficient. A child was administered medicine without obtaining medical authorization from the child's parent.

Children were not offered a fruit for breakfast. One infant was given water instead of milk at breakfast.

The use zone for the exit of one slide on the playground used by children is only four feet.

The Licensing keeping children safe poster,The Licensing Parent Notification Poster and required emergency phone numbers were not posted at the time of inspection. This deficiency was corrected at inspection when the permit holder printed and posted required postings
