-The transportation vehicle was observed with pieces of plastic wrappers, small papers, and a long sharp pin. -The Emergency medical transport and treatment authorization forms for two children who are transported were missing in the transportation binder. The Food Allergy Form was not in the transportation binder for a child with a diagnosed food allergy. Corrected at Inspection: The transportation vehicle was cleaned. The forms were placed in the transportation vehicle.
