It was determined that a caregiver pulled a child by their arm in order to get them to stand up when they were on the ground.

It was discovered that a caregiver with a provisionally eligible background status, was found to have been alone in a classroom with children for a period of less than two minutes although not allowed.

It was determined throughout the course of the DFPS Investigation the parent wasn't notified about the incident until several hours after the incident happened.

It was determined throughout the course of the DFPS Investigation a child in care was left unsupervised outside.

It was determined throughout the course of the DFPS Investigation that a child in care was neglectfully supervised.

It was determined throughout the course of the DFPS Investigation a caregiver who was responsible for the child that was left outside, was observed to have pulled out her personal cell phone and was looking at it while the children were coming inside. It was also determined that this same caregiver would use their cell phone while supervising the children in care.

The operation does not have a current fire inspection. The last inspection was conducted on 8/16/22.

Evidence gathered during this investigation show a caregiver did not use good judgment in the way she picked up a child and carried them across the room. The caregiver also did not use good judgment when handling a child after a biting incident.

There are several staff at the operation that do not have current First Aid/CPR training. A staff also needed to update their transportation training and SIDS training.

Evidence gathered during this investigation shows a caregiver swatted a child on the hand. The caregiver was fired after the incident.

The fire inspection has expired. The fire inspection expired on 12/1/21.

Children were napping in three separate classrooms with the caregiver sitting in the foyer outside of the classrooms. The caregiver could not supervise all three classrooms at one time. This was corrected during the inspection, when the children woke up from nap and were all grouped together in one class.

Evidence gathered during this investigation shows a caregiver used inappropriate discipline on a child. The investigation shows a child was hit on the bottom and put in a chair in a rough manner.

Evidence gathered during this investigation shows there was an incident regarding the use of inappropriate discipline that was not reported to Licensing within the required time frame.

One caregiver does not have 24 hours of training from her past training year. Two staff do not have current training in SIDS, Shaken Body, and Understanding Early Childhood Brain Development.
