The loose fill was measured during the inspection and found to be less than the required 6 inches.

During the inspection I observed a caregiver using her cell phone during nap time. The caregiver was not contacting a parent.

Through the investigation it was determined that a staff member was under the influence of a controlled substance while present in a classroom.

An infant was asleep in a restrictive device, and had been for more than 10 minutes according to the caregiver.

A cabinet lock in the infant room was not functioning allowing the infants access to multiple chemicals. On the playground screws were also observed to be exposed in reach of children. The lock was corrected at inspection, the caregiver added new locks onto the cabinet.

During the inspection an individual without a high school diploma or equivalent was present inside a classroom.

No loose fill was seen under the swings or the exit to the slides on the playground.

During the inspection I observed the older infant room. The room did not meet adequate lighting as it was difficult to see and my eyes had to adjust to the room.

During the walkthrough of the operation I observed a 15 month old infant asleep in a restrictive device.

The operation did not have a carbon monoxide detection system.

During the inspection 3 infant care sheets were observed to either be out of date or not available for review.

During the inspection 3 classrooms had trash cans without lids leading to the trash being accessible to the children.

During a walk through of the center I observed 2 classrooms that had the television on while it was not in use.

During the investigation it was found that the operation did have 4 recent Covid-19 cases that they did not report to CCR within the required time frame.

Five peronnel records were missing the required training.

Two blue/green mats were observed to be torn with foam exposed making them unable to be sanitized.

Bucket Seats do not have safety straps and a toddler was observed sitting in one of the seats.

Eight children's records were reviewed: Four records are missing admission dates Six records do not have address for emergency contact Three records needs additional healthcare info Four records need Emergency Care Authorization box completed with all info One record needs healthcare provider statement Two records need updated shot records.

Activity plans were not available at inspection.

After diapering, a child was placed on the floor without their hands being washed. The caregiver was notified and washed the child's hands immediately
