During the walk through inspection a child was observed getting their diaper changed and did not wash their hands after.

The operation was not completing their daily grounds and safety checklist daily before children arrive into care. Form emailed to begin using.

During the walkthrough inspection a wax warmer was turned on and in a classroom, broken metal in fence on playground, and broken plastic playhouse, leaving sharp edges.

During the walk through inspection it was observed the water fountain outside, and the bathrooms and sinks in classrooms were not being kept cleaned and sanitized.

There was debris exposed from a maintenance pile exposed to children in the outdoor play area This was corrected when the debris was picked up and placed in trash.

the sheets on infant crib matresses were not snug fit and were loose fit. This was corrected as all sheerts were taken of the matressess and replaced with snug fit sheets

A child was left in a classroom unsupervised for eleven minutes. The child was uninjured and in no apparent distress.

Several of the toddler nap mats were not labeled and not being sanitized daily.

The director's personnel file was reviewed. The pediatric CPR/FIRST AID certification is expired.

This standard was evaluated regarding a self report received and found to be deficient. Caregivers did not supervise a child in care appropriately. The child was left on the playground for approximately 3 minutes. No injuries were received.
