A caregiver wasn't aware a child in care that she was transporting did not attend public school.

The operational policy did not include information regarding allowing parents to observe the operation at any time durings hours of operation.

The monthly infant feeding sheets were not filled out and signed by a parent or physician.

A review of the Employees Who Left Employment in 2022 Survey data from January 25, 2021, revealed that the center had not reported the number of employees who left employment in 2022 by January 15, 2023, as required.

None of the 15 classrooms had written activity plans available.

One tube of diaper cream was not labeled with the infant's name. This was corrected at inspection.

5 of 28 children records evaluated did not have the authorization to obtain emergency medical care and to transport the child for emergency medical treatment.

The trash can in one of the toddler restrooms did not have a lid and contained soiled diapers. This was corrected at inspection when the trash can was replaced with a new trash can with a lid.

4 of 4 personnel records evaluated did not have the following annual trainings available: Emergency preparedness; (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication, if applicable, including compliance with 746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

A caregiver was seen on video surveillance interacting with children inappropriately and not in a positive manner. The caregiver is no longer employed at the operation.

In the infant classroom, I observed an infant lying in a crib with a bib around their neck.

During the inspection, i observed multiple classrooms with blankets over the windows that did not allow adequate lighting. Note: Corrected when the caregivers removed the blankets from the windows.

The operation did not have a signed CPSC form on file.

There was no documentation of fire drills on file.

The first aid kit in the building was missing tweezers and tape. One out of two vans did not have a complete first aid kit as cotton balls and tweezers were missing. Note: Corrected when the missing items were added to the first aid kits.

The operation did not maintain a monthly safety checklist as the last was conducted in September 2020.

There was no documentation of severe weather drills.

Two out of ten children did not have current immunization records on file.
