I observe 21 children combined for naptime and youngest was 15-months and oldest was nine years old. The difference between youngest and oldest child cannot be more 18-months difference. Note: corrected during inspection.

A review of the Employees Who Left Employment in 2022 survey data revealed that the center had not reported the number of employees who left employment in 2022.

Based on the information obtained during the interview with the children and the van driver, there was indication of personal phone usage. The van driver did admit to using the phone for personal usage while waiting in a school line.
