A 2-year-old child was left unsupervised for 15 minutes in the fox classroom.

It was found that a caregiver did not physically handle a child in care appropriately when they transitioned a child by one arm three separate times when they dropped their weight to the ground.

Four infants ages 13 - 17 months old were observed seated at feeding tables. The required safety straps were not fastened while in use. Caregivers were asked to fasten the straps during the inspection.

It was observed that one caregiver did not wash the hands of an infant after a diaper change.

It was found during the investigation that one child was left unsupervised in the hallway of the building for about 4 minutes.

The operaiton did not report a situation placing a child at risk to licensing within the required time frame.

A parent was not notified of a situation placing their child at risk in a timely manner.

During the inspection it was found that there was a situation that involved a child being left in their classroom without supervision for approx 3 min. The caregiver involved in this situation is no longer working at the operation and supervision at the operation was observed to meet compliance during the investigation inspection.

During the inspection it was found bibs, mats, and spoons used by infants in care during meal times were washed in the handwashing sink. This was corrected at inspection when management informed all operation staff of new system being implemented which is to place meal items in sanitation bins to be sanitized in workroom.

A caregiver was observed changing a child's diaper and failing to wash their hands as well as the child's. This was corrected at inspection when the caregiver washed their hands and the child's hands.

It was found that a caregiver does not physically handle the children in care appropriately.

It was found that a caregiver left a child on the playground alone.

A health statedment from a medical professional was missing from a child's file.

The operational policy was missing a section regarding infant safe sleep.

The loose fill under a play structure did not meet the 6 inch requirement.

The operation failed to update the medication authorization forms before administering medication to a child in care.

The operation failed to obtain authorization from a parent prior to administering medication.

The medication authorization form was not properly documented using the full name of the employee who administered the medication.

The 2A classroom with a specified age of two-years had fifteen children and one caregiver. A second caregiver came into the room to meet ratio requirements.

A caregiver failed to use good judgement and self control to properly communicate with children in care.

A staff was heard yelling at a child during the inspection. This was corrected at inspection when the staff was reprimanded and received verbal redirection for the behavior.
