While doing a walkthrough, 1 class was observed serving snack directly on the table. This was corrected when CCR asked them to put the snack on a napkin.

This standard was evaluated as part of the investigation and found to be deficient. Two caregivers admitted to placing a child in a restrictive device for discipline purposes.

While doing a walkthrough, one class was sitting in a bucket table with 6 children not straped in. This was corrected when CCR asked the caregivers to buckle the children and they did.

This standard was reviewed during an investigation and found deficient. The operation did not have current CPR/First Aid training.

The operational policy did not include information regarding vaccine preventable diseases for employees.

The operation could not provide documentation that a local health department is not avaiable in the area.

There was an employee present in the operation with a group of 7 children age 2-3 that did not have an eligible background check. This was corrected when the caregiver was sent home.

There were 3 children files that did not have addresses for emergency contacts on file.

This standard was reviewed in relation to the self-reported allegations and found to be deficient. Video evidence shows that a caregiver pulled two children by their arms to move them to a different location. The caregiver is no longer employed at the operation.

During HASA inspection, two training records were unavailable for review.
