A small slide on the playground was broken. Note: The slide was removed during the inspection.

Based on video footage, a caregiver briefly stepped out of the classroom leaving the other caregiver over ratio by three children.

Two classrooms had cleaning supplies stored under the sink cabinet. Note: This was corrected when the cleaning supplies were moved out of the reach of the children.

The fire extinguishers were last serviced February 2024.

Two caregivers did not use good judgement when there was a delay with checking on a child who had fallen until the child turned around and they saw the child was bleeding. Their initial responses to the fall were inappropriate.

Two infants were observed sleeping in the crib with blankets. Note: The blankets were removed when the inspector entered the room.

The operational policies lacked information regarding the criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play. Note: This was corrected when the director added this to the policy.

The operation failed to communicate to a parent in a timely manner that their child had consumed feces, placing the child at risk of serious health consequences due to the child not receiving medical evaluation.

A caregiver demonstrated poor judgment when she dragged a child by their arm several feet while they resisted instead of stopping and picking up the child correctly, resulting in a serious injury to the child's elbow.

The operation failed to properly document a situation where a child was placed at risk due to ingesting feces.

The operation failed to timely notify the parents of the children involved that their children exited the building unsupervised. This incident occurred around 11am and parents were not notified until pick up time (after 5pm).

Based on information gathered, two children exited the building and were left unsupervised for approximately 1 minute 41 seconds.

One child was napping blocking an emergency exit. NOTE: This was corrected at inspection with the child being moved to another place.

Infant Room and Prek Classroom did not have the current Activity Plan posted. NOTE: This was corrected at inspection with the currrent activity plan being posted in both classrooms.

Caregiver purse was observed sitting on the ground accessible to the children posing a safety hazard. Electrical Cord in the Pre-K Classroom was observed not being secured to the wall posing a safety hazard.

Children were observed not having their hands washed after coming inside from outdoor play.

Infants did not have the monthly feeding schedule complete and available for review at the time of inspection.

Based on information gathered, caregivers are calling or referring to some children by demeaning nicknames, therefore children are not being respected or accepted.

Based on information gathered, caregivers are leaving the classroom when they are in ratio to visit with other staff in the office, which in result is leaving one caregiver with too many children.

The operation failed to notify licensing within two days that a caregiver tested positive for COVID-19. This was corrected when it was reported to licensing.

Based on information gathered, the director is not fulling her responsibilities and addressing concerns or seeking resolution when issues are being brought to her attention.

Based on information gathered, caregivers are hurrying children to eat during mealtimes and not allowing them to finish their meals.

Caregivers did not demonstrate good judgment when they threw children's food away prior to them being done eating, resulting in children being hungry and upset.

Based on information gathered, a caregiver kicked a child who was in time-out. Another caregiver has forcefully pulled children by their arm.

A caregiver left a toddler in a pac-n-play for at least 25 minutes as a form of discipline.
