Two children in care did not have immunization records on file for review.

A child with a diagnosed food allergy, did not have a food allergy plan on file for review.

It was determined that proper supervision was not maintained while children were playing on the playground and a child was able to get out of a fence on the playground, into an unfenced area.

The operation did not have a current health inspection on file.

The child safety alarm was tested for the transportation vehicle and it was not observed to be working correctly.

The operation did not have the latest inspection form posted. The last inspection form posted was from 2022.

There was a caregiver that did not have in person CPR training with utilization of a manikin.

During an investigation, it was found that caregivers did not supervise a child in care when the child was left outside without adult supervision.

During an investigation it was found that children under the age of 5 years old were playing in an area that was not fenced in.

During an investigation there was sufficient evidence to support a child was neglected.

There was 1 classroom that was out of ratio. The classroom had 7 children with one caregiver in the room, while the other caregiver was throwing the trash away. The other caregiver came back into the classroom to bring the deficiency into compliance.

It was determined the operation did not notify Child Care Regulation within 48 hours as required. The incident occurred on 3/29/23 and Child Care Regulation was not notified until 4/11/23.

It was determined during the interviews that a caregiver inappropriately disciplined a child by popping the child on the bottom, swatting at his hand and yanking the child off the table.

The operation had not completed/documented a fire drill since February 2021.

There were 4 children observed in a bucket seat that manufacturer required safety straps and the safety straps were not observed to have been put on the chairs in order for the children to be fastened.

There were 7 infants in care that did not have an updated infant feeding instructions on file.

There were 5 caregivers that did not have a high school diploma or equivalent on file available for review.

It was observed and determined through the investigation, the outdoor play equipment layout does not allow for adequate supervision of children.

Two caregivers, in 2 separate classrooms were observed on their cell phones while supervising a group of children during nap time.
