St. Helena's Early Enrichment Pre-school

A child with a food allergy plan on file did not contain the parent's signature on the plan.

Two out of five caregiver files evaluated did not have a current first aid certificate on file.

A caregiver was observed in the hallway and answered the front door, leaving ten 3-4 yr olds napping in the classroom unattended.

The operation did not conduct a fire drill for the month of January.

There was a metal canoe on the lower playground that had jagged points on the seat that could potentially cut a child's finger or hand. NOTE: A work order was submitted during the inspection to have the canoe removed.

Monthly building and equipment maintenance checklists have not been conducted.

Two rescue medications did not contain a prescription box or label.

Six out of nine booster chairs used did not contain safety straps.

Children were observed eating morning snack outside on the concrete.

Three children with medication at the operation did not contain an allergy plan on file. One child's allergy plan was not signed by the parent.

The operation had a change in their controlling persons in March 2023 and had not updated this information.

One out of 4 staff files evaluated did not have any training certificates for the evaluation year of 2021-2022.

There were two caregiver purses on the classroom counter accessible to children. There was a container of Clorox wipes on a counter accessible to children. There were television and radio cords hanging in two classrooms within the reach of the children.

During morning drop off time, between 8:30-8:45 am, there were 21 children, ages 2-4 years, in the enrichment room that has a maximum capacity for 11 children.

One child with medicaiton on file did not have the original medicaiton container with the prescription.

Room E and the 3-4 year room both had cords hanging from a mounted television and Christmas lights that were within reach of the children. Upper playground had exposed wires on the ground leading from the building to the electrical and cable boxes accessible to the children.

The upper playground had 2 missing fence boards creating a gap of over 3 inches that could cause possible entrapment. There was also a gap of more than 3 inches where slats of the fence do not meet the ground and create an area where children can attempt to climb under resulting in exiting to the street. Lower playground has a playhouse where the front of the house has come unattached from the house causing a possible entrapment or pinch hazard.

The upper playground had a section of a wall that is used as part of the fenced area that was missing and does not meet the 4 foot height requirement. This creates the ability for children to climb over and exit the playground.

The operation's vehicle fire extinguisher was last inspected in 12/2020.
